Over the course of the 20th century, smoking killed about 100 million people, most of them in today's rich countries. The health burden of smoking is shifting from high-income to low- and middle-income countries, and some estimates suggest that 1 billion people could die from smoking in the 21st century. According to the GBD, more than 8 million people died prematurely as a result of smoking in 2017. To break this number down, 7 million people died from smoking itself.

The debates on Tobacco Harm Reduction (THR) as a public health solution that recognises the harm caused by combustible cigarettes and aims to minimise the health impacts of cigarette smoking by encouraging those adult smokers who would otherwise continue to smoke to switch completely to scientifically substantiated, reduced-risk alternatives ought to be explored in our contemporary society. To many there's nothing like THR and to others there exist the THR.

The Concept of THR needs to be better discussed to increase knowledge and to be understood by consumers and those in the tobacco control communities.
